Candidate Filing Packets

Candidate Filing Paperwork

All candidate filing packets are available online through the Secretary of State’s website.

Follow the prompts for your office to download the appropriate filing packet.

Important: All filing forms were updated in June 2025 to reflect changes in state law. Previous versions of the filing paperwork will not be accepted.

Before filing, please review the following requirements:

Complete all applicable sections of the online fillable filing form.
If notarization is required, all required signatures must be acknowledged by a Notary Public, and the notary’s commission number and expiration date must be included. The Clerk’s office cannot notarize these forms. 
Filing fee checks should be made payable to the Warren County Clerk’s Office.
Candidates may file in person or by mail. Filing documents must be received by the applicable filing deadline.

Candidate Responsibility

Pursuant to KRS 118.165(2), the County Clerk’s Office reviews only the face of the filing form and declaration for completeness.

The Clerk’s Office does not verify a candidate’s:

Qualifications for office
Party affiliation
Witness signatures
Residency requirements
Any other information contained within the filing documents

It is the candidate’s responsibility to ensure all information contained in the filing documents is accurate and complete.

Campaign Finance Requirements (KREF)

Campaign Finance Filings

Every candidate, including those filing for an exemption, must electronically file a Statement of Spending Intent and Appointment of Campaign Treasurer (KREF 001).

Candidates must create an eFiling account through the Kentucky Registry of Election Finance (KREF).

Candidates who wish to begin campaign activities before the official filing period may file a Letter of Intent with the Kentucky Registry of Election Finance.

Financial Interest Statement

In accordance with KRS 65.003, Warren County and each incorporated city within the county have adopted ethics ordinances requiring certain candidates to file a Financial Interest Statement.

Candidates for the following offices are required to file this statement within twenty-one (21) days after submitting their filing paperwork:

County Judge/Executive
Fiscal Court
County Clerk
County Attorney
Sheriff
Jailer
Coroner
Surveyor
Constable
Mayor
City Commissioner
